what is dental impaction?

dental impaction is a condition when a teeth/tooth fails to erupt in its dental chronological time. The impacted teeth usually stay within the bone of the individual throughout his/her lifetime, this impacted teeth may produce pain or may sit idle without any symptom. 

What are teeth that mostly get impacted?

The wisdom teeth or the 3rd molar(the last two teeth on both the end of the jaw) is the most common teeth that get impacted, this is because the wisdom teeth are the last tooth to erupt and when all the teeth in the jaw have occupied the space completely the last erupting teeth get impacted in the bone.

The second most common teeth to get impacted is upper canine, this is because the upper canine teeth have the longest journey of eruption, if the erupting teeth space is narrow the teeth get impacted.

Other teeth also get impacted in very rare conditions.

Other reason for tooth impaction

  1. Retained deciduous teeth,
  2. Trauma during the eruption,
  3. Extra teeth than normal,
  4. Crowded teeth,
  5. Childhood caries,
  6. Early removal of milk teeth.

What is the treatment for impacted teeth?

Tooth Removal is usually suggested but in few conditions, operculectomy( removal of gums placed over the impacted teeth.) will help in normal eruption and orthodontic treatment will some times help in a normal eruption of teeth.

The treatment varies depending on the teeth and its position.

WHAT IS Impacted Wisdom Tooth?

Wisdom tooth impaction is a condition where your last molars of the arch, called 3rd molars

  •         May not have erupted in their position.
  •         May have a tilted because of insufficient space.
  •         Maybe completely not visible in your mouth as they are present inside the tissue.


Wisdom tooth impaction is common in people age between 18 – 30 years and sometimes above.


Wisdom tooth impaction mostly presents as pain in the region of your molars or behind them. Most of the time they get resolved immediately after the impacted teeth are removed.

Sometimes, you find a swelling associated with pain. This happens when you neglect the initial symptoms like mild discomfort or pain. Swelling usually occurs because of the accumulation of pus. The size varies from small pea-sized to covering the entire side of the face depending on the severity of the condition.

There are instances where the impacted tooth inside the bone may hit the adjacent teeth and lead to cavity formation in the adjacent teeth. When neglected, both your teeth get damaged and you will have to undergo some lengthy treatment procedures to get your teeth functioning.


When you visit your dentist with the above symptoms, your dentist will check your oral cavity and take an X-ray if required. Then your treatment will be planned according to the severity of the condition.

Let your dentist know if you have conditions like high blood pressure, diabetes, any bleeding disorders, heart diseases, and the present medication you are taking for any condition. This is important for the treatment plan and post-operative medications.

There are many types of tooth impaction but we commonly have 3 types:

  1.       Angular
  2.       Horizontal
  3.       Vertical

Angular type of impaction is most common and it presents as tilting of the molar towards any side in the jaw.

A horizontal type of impaction is a condition where the tooth is completely inside the bone and is pushing the adjacent tooth the extent where you feel pain in that tooth. This is considered as one of the worst types of wisdom tooth impaction as it presents with many complications.

Vertical type of impaction is something where your teeth have enough space to grow without any tilt but the bone and gums are so thick that they hinder their eruption.

Your dentist will first numb the area of pain with a local anesthetic. Then they will reflect all the soft tissues around the impacted tooth and if required, he/she will trim the bone around the tooth to make the removal easier and pain-free. Finally, all the tissues will be approximated and sutures are placed.


Your dentist will give you a set of instructions to help quick healing of your wound. Let’s check the common instructions given:

  •         You shouldn’t spit for at least 24 hours.
  •         Take the medications as prescribed by your dentist regularly.
  •         Avoid hot and spicy foods for a week.
  •         Don’t rinse your mouth vigorously.
  •         Brush your teeth gently.
  •         Apply ice pack on the swelling, if you have any.


There are no such home remedies that will cure the symptoms of wisdom tooth impaction. But some of the following help you to get some relief from the pain:

  •         Warm salt water gargle
  •         Apply ice pack if you have any swelling
  •         Numbing gels that are available over the counter.
  •         Apply clove oil or place a clove bud near the pain region.


 For immediate relief of pain, you can use paracetamol or ibuprofen or ketorolac tablets. But remember that the only permanent cure is to get the appropriate treatment done by your dentist. As long as you postpone your dentist’s visit, the severity of the condition will be increased.

Should we have to replace the wisdom teeth after removal?

Wisdom teeth even if it has erupted is an idle tooth, it is not used in chewing food, speech, or any other normal functions, removal does not cause any tooth shifting or facial disfiguration, hence replacement is not necessary.

The extracted socket may sometimes remain hollow, placing a bone graft in the extracted socket will avoid it.
